Frequently Used Forms & References
The following are forms frequently used or required. Nearly all can be completed electronically and sent via email to the Central Office. The most common forms used by student members of the fraternity are collected in the NCR excel files personalized for each chapter and colony, and found at the very bottom of this page. This NCR excel file contains worksheets with the following forms: Officer Election Report, Membership Status Change Report, Co-Op Report (also used for member on "study abroad" or called to military duty), Initiation Report, Jewelry Price List, and Annual Report (Chapter's Only).

Request grant of alumnus status for undergraduate member providing the following: no outstanding financial obligation to the chapter, no unfulfilled contractual obligations to the chapter (if applicable), the completion of at least two semesters of active membership, a significant lifestyle change preventing the fulfillment of fraternity duties and obligations, and submission to the Executive Director's recommendation.

Bylaws, Chapter II, Part B, Section 11. An undergraduate member who has been a student member of the Fraternity for at least 4
years may elect to assume alumnus membership in this Fraternity if his account is paid in full, by submitting application request and fee to the Executive Director in the manner prescribed by the Executive Council. Such a status change is only effective once approved by the Executive Director, and once approved, such a member shall have all the rights of alumnus membership.

This form is intended to serve as a checklist when carrying out disciplinary action against a member (see numbered items). You should consult the Constitution and Bylaws for more specific instructions regarding the proper manner in which to file charges and hold a trial. In addition, you should refer to the Discipline Section of your Chapter Officers' Manual (below) for advice on how to prevent the need for such action.

This form is to be completed only for those previously granted Premature Alumnus Status and requesting to resume student member status, with the understanding that justification will be provided and the semiannual dues obligation for the current semester must be met if student member status is resumed on or before the semiannual dues deadline (November 1/ March 15).
